Reply form

You only need to complete the Reply form if there are any errors on your statement. If the error(s) affect your pension benefits your pensions administrator will send you a new statement. If the changes do not affect the workings of your pension benefits, e.g. spelling mistakes, nominee details or marital status, they will show up in your next annual statement.

Death benefit nominations - important note! If you see from your statement that you need to change your death benefit nomination, you must complete a new death benefit nomination form.

Your pensions administrator cannot change your nomination based on notes you make to your statement; they will have to send you a nomination form to complete if you do this. Please download the appropriate form under ‘Publications rules and forms’ and send it to your administrator.
